HOW DO I....

REGISTER WITH THE PRACTICE?

New patients wishing to register will not be discriminated against on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or mental condition.

If you live within our catchment area you may register with the practice. You should come to reception with your medical card. If you do not have this we have a form you may complete.Patients registering with this practice will be expected to provide proof of ID and residency, complete a health questionnaire and, if over 15, provide a urine sample for routine testing and use the blood pressure monitor in the waiting room. New patients who are pregnant, have a chronic disease or are on routine medication should make an appointment with the appropriate health care professional.
